Proof of address will be required i.e. utility bill, bank statement etc. A form will also be needed to be filled after which an appointment with the nurse will be booked.
All information is required to be updated. This enables the surgery to contact the patient in the event of an emergency. This is the responsibility of the patient.

We operate a zero tolerance policy. Violence and abuse against any staff member will not be tolerated.

Every appointment you make with the receptionist and every consultation with the Doctor or Nurse is confidential. We will not inform anyone without your prior permission.
